1. Call the Blues Front Office at 314.622.2566.
2. Pay the $35 annual fee ($70 for two years) by credit card or check - of which a portion benefits the St. Louis Blues 14 Fund, the Blues’ charitable trust.
3. Receive the Emblem Use Authorization Form from the Blues and take it to your local license office where you will pay a $15 one-time personalized license plate fee and your annual vehicle registration fee.

1. What forms do I need?
In order to get personalized Blues license plates, you need to submit the following forms to the Missouri Department of Revenue:
- St. Louis Blues Emblem Use Authorization Statement: Provides documentation that you have already paid the $35 annual fee to the Blues.
- DOR Form 1716: Gives you the opportunity to specify what you would like on your personalized license plates.

2. How much does it cost?

3. I just registered my car. Will I have to register it again?
No. The St. Louis Blues plates will simply replace your current plates. Registration stickers will come with your new plates. You will need to turn in your current plates upon receiving your new Blues plates.
4. I already have personalized plates. Will I lose my plate?
No. Your personalized plate is yours to keep unless you fail to renew it each year. Your current personalized plate will simply be placed on a Blues plate providing it stays within the parameters of the six-digit limit.
5. How long is the form good for? Does it expire?
The Emblem use Authorization Form will not expire and is valid from the time it is submitted to the time the plates need to be renewed.
6. Will the Blues contact me when my plates are about to expire?
No. The Missouri Department of Revenue will send license renewal notices when plates are about to expire. Please allow enough time to receive your Emblem Use Authorization Form. The end of the month is busier than the beginning, so please plan ahead.
